le d\u00e9partement de la sant\u00e9 de Tulsa est l&#039;agence de sant\u00e9 publique principale pour plus de 675 000 r\u00e9sidents du comt\u00e9 de Tulsa, dont 13 municipalit\u00e9s et quatre zones non constitu\u00e9es en soci\u00e9t\u00e9. L&#039;agence est l&#039;un des deux d\u00e9partements de sant\u00e9 locaux autonomes de l&#039;Oklahoma, avec une comp\u00e9tence statutaire en mati\u00e8re de sant\u00e9 publique dans tout le comt\u00e9 de Tulsa et la ville de Tulsa. La mission du THD est de prot\u00e9ger et de soutenir les communaut\u00e9s du comt\u00e9 de Tulsa dans la poursuite de leurs objectifs de sant\u00e9, afin d&#039;aider les gens \u00e0 atteindre leurs objectifs de sant\u00e9. Le THD a \u00e9t\u00e9 l&#039;un des premiers d\u00e9partements de sant\u00e9 aux \u00c9tats-Unis \u00e0 recevoir une accr\u00e9ditation nationale par le biais du Public Health Accreditation He continues to evaluate the various departments while reviewing our processes to identify areas for improvement, with a particular focus on enhancing our finance department and billing practices. <br \/>Additionally, Dr. Dart introduced the Board's newest member, Dr. Carolina Hernandez.<\/p><p>5) THD Mission Moments \u2013 Office of Maternal &amp; Child Health <br \/>Healthy Start Program \u2013 My Experience with Healthy Start \u2013 Dorothea Fullard<\/p><p>Dorothea Fullard, with the Healthy Start Program, was a participant in the Healthy Start program beginning in 2019 during her first pregnancy. Although she had prior experience working in OBGYN, becoming a mother for the first time introduced a level of uncertainty and overwhelming information that she was not prepared for. When Miss Latoya from Indian Health contacted her and enrolled her in the Healthy Start program, it became a turning point in her journey. During a time when advice, medical information, and opinions came from every direction\u2014family, social media, and even her own clinical background, Healthy Start provided clarity. Miss Latoya helped quiet the noise and empowered Dorothea with practical knowledge, such as how to listen to her baby, how to communicate and bond through reading and singing, and how to understand what was happening during her prenatal appointments. Miss Latoya became not only a case manager, but a trusted support person and, in many ways, a part of the family. After giving birth, Dorothea became critically ill with COVID-19 and required a ventilator for five days postpartum. When Dorothea finally returned home, she lacked basic resources and was unsure how to obtain essentials like formula or a breast pump. Through her case management work, Miss Latoya guided Dorothea through those challenges and ensured that she had access to the support she needed during an incredibly vulnerable time.<\/p><p>Dorothea is deeply grateful for both the Healthy Start program and the compassionate, dedicated individuals who lead it. This program played an essential role in helping navigate one of the most significant periods of Dorothea\u2019s life. Dorothea continues to recommend Healthy Start to anyone she meets who is pregnant, because the level of care and support it provides is truly exceptional.<\/p><p>6) Current Business | Information Items &amp; THD Reports<\/p><p>A. FY 2027 THD Preliminary Budget \u2013 Dr. Bruce Dart, Executive Director <br \/>Dr. Dart outlined the proposed budget of $44.6 million for the fiscal year 2026-2027, noting a slight decrease from the previous year. The budget draws from ad valorem taxes, permits, miscellaneous income, local grants, state grants, and federal grants, with an emphasis on the need to diversify funding sources due to unpredictable federal support.<\/p><p>Several grants have ended or have been reduced, including the Strategic Partnership Framework (substance abuse grant), the TSET tobacco program, the TANF grant for the prep program, and the OSU behavioral health workforce development grant. The CDC Public Health Infrastructure Grant saw a 33% increase, but its future is uncertain, while the HUD Lead Hazard Control Grant continues to be successful.<\/p><p>Capital expenditures include $800,000 for an air handling unit, fire alarm system upgrades, AV hardware, new vehicles, HVAC replacements, backup server, lab equipment, and security room expansion. The budget proposes a 2% cost-of-living adjustment for staff and aims to cover a 5% increase in health insurance rates, with a focus on maintaining at least six months of cash on hand.<br \/>Staffing has decreased from a peak of 360-365 to around 350 due to grant reductions. The department plans to build a true reserve fund for emergency preparedness and monitor cash on hand more closely, with 69% of employees funded by the budget and a majority of costs being fixed.<\/p><p>B. Point sur la session l\u00e9gislative \u2013 Scott et Alexandra Adkins <br \/>Scott and Alexandra Adkins shared a legislative update with the Board. The governor signed Senate Bill 1177, the general appropriations bill, with a 2.92% decrease for the State Department of Health. Cuts were limited to one-time funds, avoiding across-the-board reductions for agencies. The House budget portal provides detailed breakdowns of agency requests and appropriations.<br \/>Resolution 39 proposes to slow property tax growth by reducing caps for homestead and agricultural land and introduces a senior freeze for those 65 and older. This measure will appear on the August runoff ballot and may impact future local budgets.<\/p><p>House Bill 4440 and HJR 1067 propose changes to Medicaid expansion funding, with triggers based on the federal match rate. The first would move expansion from the Constitution to statute, while the second could eliminate expansion if the match dropped. Both may appear on the November ballot, creating uncertainty for healthcare funding.<\/p><p>Alexandra is still watching the bills for rural health transformation, medication continuity for children in state custody, maternal health initiatives, foster care support, eviction timelines, and the definition of family resource centers. Immigration status checks for benefit recipients were clarified to protect children\u2019s access to services.<\/p><p>C. Healthy Start\u2019s Strategic Plan \u2013 Robin Daniels <br \/>Robin Daniels presented the Healthy Start program\u2019s strategic plan. There are four core areas, performance benchmarks, outreach activities, consortium collaboration, and upcoming initiatives. Dorothea Fullard shared her client experience and board members discussed alignment with city priorities and future quality improvement efforts.<\/p><p>The Healthy Start strategic plan was shared with the HRSA program officer, who recognized it as the first of its kind among all Healthy Start programs and requested it to be shared as a model for other recipients and HRSA leadership.<br \/>The program focuses on case management and clinical services, group-based education, the fatherhood program, and the consortium. Performance goals include enrolling 250 new pregnant clients, serving 175 infants\/toddlers\/non-pregnant clients, and maintaining case manager caseloads of 45-50 clients. Year-to-date data shows strong progress towards these goals.<\/p><p>Healthy Start staff have visited 40 locations within targeted zip codes, built an outreach team, and established relationships with pediatric clinics. Ongoing training and referral incentives are used to boost enrollment and participation in case management and group education.<\/p><p>The consortium merged with SHIFT, expanding membership to 110 and planning to form focus groups on mental health, healthcare, and nutrition. Upcoming events include a Medicaid enrollment fair, a campaign on early prenatal care, and a family-oriented zoo trip.<\/p><p>Plans include implementing a quality improvement process focused on client retention and satisfaction through quarterly surveys and continuing to spotlight local resources and develop focus group-driven solutions for community health challenges.<\/p><p>7) Current Business | Action Items <br \/>A. Cette \u00e9tude portait sur un incident r\u00e9cent\u00a0: un r\u00e9sident avait signal\u00e9 avoir \u00e9t\u00e9 expos\u00e9 \u00e0 la fum\u00e9e secondaire dans son appartement. Le service a collabor\u00e9 avec les complexes d&#039;appartements et les entreprises locales pour \u00e9laborer et mettre en \u0153uvre des politiques antitabac, en soulignant les b\u00e9n\u00e9fices pour la sant\u00e9 publique et les \u00e9conomies r\u00e9alis\u00e9es par les propri\u00e9taires en mati\u00e8re de pr\u00e9vention des incendies et de r\u00e9duction des co\u00fbts d&#039;entretien. L&#039;impact est plus large\u00a0: le plaidoyer a \u00e9t\u00e9 couronn\u00e9 de succ\u00e8s pour des politiques antitabac dans plusieurs villes comme Tulsa, Owasso, Collinsville, Sand Springs et Jenks, couvrant les propri\u00e9t\u00e9s et les v\u00e9hicules municipaux, et des demandes de subventions ont \u00e9t\u00e9 d\u00e9pos\u00e9es pour des initiatives antitabac. L&#039;audit s&#039;est conclu par une opinion sans r\u00e9serve concernant les \u00e9tats financiers, les contr\u00f4les internes et la conformit\u00e9 aux programmes f\u00e9d\u00e9raux, aucun probl\u00e8me ni d\u00e9saccord n&#039;ayant \u00e9t\u00e9 relev\u00e9 au cours du processus.<\/p><p>Trois grands programmes f\u00e9d\u00e9raux ont \u00e9t\u00e9 test\u00e9s et ont tous d\u00e9montr\u00e9 une documentation justificative satisfaisante et une conformit\u00e9 irr\u00e9prochable, sans qu&#039;aucune difficult\u00e9 ne soit rencontr\u00e9e. De plus, l&#039;actif net a augment\u00e9, tant pour le fonds que pour les \u00e9tats financiers consolid\u00e9s du gouvernement, ce qui indique une situation financi\u00e8re favorable pour l&#039;organisation.<\/p><ol><li><strong>Point sur la session l\u00e9gislative \u2013 Scott et Alexandra Adkins \u00a0<\/strong><\/li><\/ol><p>Scott et Alexandra Adkins, lobbyistes aupr\u00e8s des instances l\u00e9gislatives, ont pr\u00e9sent\u00e9 un bilan complet des projets de loi en cours relatifs \u00e0 la sant\u00e9 publique, de la dynamique l\u00e9gislative et des strat\u00e9gies de plaidoyer. Alex a notamment mis en lumi\u00e8re des projets de loi concernant Medicaid, l&#039;immigration, les imp\u00f4ts fonciers, le tabac, le cannabis, les vaccins, la sant\u00e9 maternelle et la politique alimentaire, tout en abordant l&#039;impact du contexte politique \u00e9lectoral.<\/p><p>Ils ont constat\u00e9 que certains projets de loi exigent une v\u00e9rification du statut d&#039;immigration des b\u00e9n\u00e9ficiaires de Medicaid et du SNAP. Des amendements r\u00e9cents limitent ces v\u00e9rifications aux b\u00e9n\u00e9ficiaires eux-m\u00eames, et non \u00e0 leurs parents, ce qui facilite l&#039;acc\u00e8s aux services pour les enfants dans le besoin. Les d\u00e9bats en cours portent sur l&#039;abrogation de la taxe fonci\u00e8re, les exemptions de r\u00e9sidence principale et les contraintes budg\u00e9taires. Ces enjeux soulignent l&#039;importance de maintenir le financement de la sant\u00e9 publique et des autres services essentiels dans un contexte budg\u00e9taire difficile.<\/p><p>La discussion a \u00e9galement port\u00e9 sur plusieurs projets de loi concernant le tabagisme passif, la r\u00e9glementation du cannabis et l&#039;extension des politiques antitabac. Il s&#039;agissait notamment d&#039;inclure le cannabis dans la r\u00e9glementation antitabac et de r\u00e9glementer les boissons et produits comestibles \u00e0 base de cannabis afin de prot\u00e9ger les enfants et les animaux domestiques. Elle a d\u00e9taill\u00e9 les efforts de pr\u00e9vention, les partenariats, les statistiques r\u00e9centes de prise en charge des cas et les r\u00e9ponses concert\u00e9es aux cas actifs dans la communaut\u00e9. Felisha a \u00e9voqu\u00e9 un exemple r\u00e9cent illustrant leur mission en action\u00a0: la collaboration avec les \u00e9coles publiques de Tulsa et celle avec la soci\u00e9t\u00e9 de dialyse suite \u00e0 l\u2019identification d\u2019un cas de tuberculose active. Ce cas aurait pu avoir des cons\u00e9quences importantes, mais gr\u00e2ce \u00e0 un d\u00e9pistage pr\u00e9coce et aux partenariats mis en place, tous les membres de la population ont pu \u00eatre test\u00e9s rapidement. La participation au d\u00e9pistage a \u00e9t\u00e9 bonne et les infections, qui auraient pu se propager rapidement, ont \u00e9t\u00e9 ma\u00eetris\u00e9es. Ce cas montre que la sant\u00e9 publique est plus efficace lorsqu\u2019elle repose sur la confiance, la coordination et le partage des responsabilit\u00e9s. L\u2019\u00e9quipe de six personnes charg\u00e9e de la lutte contre la tuberculose et la collaboration sont essentielles \u00e0 son fonctionnement. La priorit\u00e9 est donn\u00e9e non seulement aux soins et \u00e0 la s\u00e9curit\u00e9 de ses patients, mais aussi \u00e0 la s\u00e9curit\u00e9 de chacun. Cette approche d\u2019\u00e9quipe garantit la continuit\u00e9 des soins, la responsabilisation et la r\u00e9silience dans la gestion de cette maladie complexe et de longue dur\u00e9e. Cette d\u00e9marche d&#039;am\u00e9lioration continue permet \u00e0 la clinique de s&#039;adapter \u00e0 l&#039;\u00e9volution des besoins tout en maintenant des normes de soins \u00e9lev\u00e9es.<\/p><p><strong>Actualit\u00e9s | Informations et rapports THD<\/strong><\/p><ol><li><strong>D\u00e9cryptage de la \u201c super grippe \u201d \u2013 Madison Brillhart <\/strong><\/li><\/ol><p>Madison Brillhart, superviseure en \u00e9pid\u00e9miologie, a pr\u00e9sent\u00e9 un rapport complet sur la saison grippale actuelle, incluant les tendances nationales et locales, les taux d&#039;hospitalisation, l&#039;impact de la souche H3N2 ' Super K ' et les derni\u00e8res informations sur l&#039;activit\u00e9 li\u00e9e \u00e0 la COVID-19, au VRS et \u00e0 la rougeole.<\/p><p>Activit\u00e9 grippale nationale \u2013 La semaine derni\u00e8re, l&#039;activit\u00e9 grippale est rest\u00e9e \u00e9lev\u00e9e, mais en baisse (taux de positivit\u00e9 au test TP3T de 18,61\u00a0% contre 24,61\u00a0% la semaine pr\u00e9c\u00e9dente).
