Las escuelas primarias del condado de Tulsa que est\u00e9n interesadas en asociarse con THD School Health deber\u00e1n completar una evaluaci\u00f3n anual de preparaci\u00f3n para la salud escolar. Las escuelas asociadas tambi\u00e9n deben tener un Memorando de Entendimiento (MOU) del distrito. La evaluaci\u00f3n nos permite comprender el clima de su escuela y adaptar nuestros servicios en consecuencia. Una vez que se haya recibido y revisado la evaluaci\u00f3n, un representante de Salud Escolar se comunicar\u00e1 con usted para discutir posibles opciones de programaci\u00f3n. Gracias por su inter\u00e9s en asociarse con THD School Health.<\/p><h2>Fechas de asociaci\u00f3n del a\u00f1o escolar<\/h2><ul><li>Ya est\u00e1 abierto el plazo de evaluaci\u00f3n para el curso escolar 2026-2027.<ul><li>La evaluaci\u00f3n se abre el 1 de mayo de 2026 y cierra el 14 de agosto de 2026.<\/li><li>Reuniones virtuales de planificaci\u00f3n: del 1 de mayo al 21 de agosto de 2026<\/li><\/ul><\/li><\/ul><p>No se aceptar\u00e1 ninguna evaluaci\u00f3n enviada despu\u00e9s del 14 de agosto de 2026. Las reuniones de planificaci\u00f3n deben finalizar antes del 21 de agosto de 2026 para que podamos colaborar con su escuela para el a\u00f1o escolar 2026-2027.<\/p><h2>Sigue estos pasos<\/h2><p><strong>Paso 1:<\/strong>Revise la versi\u00f3n en PDF de la evaluaci\u00f3n que le enviamos por correo electr\u00f3nico desde nuestro programa de salud escolar para asegurarse de que se haya recopilado toda la informaci\u00f3n necesaria antes de comenzar la evaluaci\u00f3n en l\u00ednea. La informaci\u00f3n necesaria para completar la evaluaci\u00f3n corresponde al a\u00f1o escolar 2025-2026. Si no puede responder alguna pregunta, simplemente escriba N\/A. Le recomendamos colaborar con el director, el subdirector, los coordinadores de nivel de grado, el profesor de educaci\u00f3n f\u00edsica, el\/la enfermero\/a o auxiliar de salud, el\/la consejero\/a y el\/la coordinador\/a comunitario\/a o enlace con los padres para recopilar la informaci\u00f3n y los datos necesarios para la evaluaci\u00f3n.<\/p><p><strong>Paso 2:<\/strong>Seleccione las opciones de programaci\u00f3n preferidas.<\/p><p><strong>Paso 3:<\/strong>Complete y env\u00ede la Evaluaci\u00f3n de preparaci\u00f3n para la salud escolar de THD al final de la p\u00e1gina.<\/p><p><strong>Etapa 4:<\/strong>Nuestro equipo se comunicar\u00e1 con usted para programar una reuni\u00f3n de planificaci\u00f3n virtual.<\/p><p><strong>Paso 5:<\/strong>Re\u00fanase
