Nuestros trabajadores comunitarios de salud son la clave para llegar a las poblaciones desatendidas, brind\u00e1ndoles la atenci\u00f3n que necesitan y sirviendo como puente para acceder a los servicios necesarios. Utiliza una intervenci\u00f3n centrada en trabajadores de salud comunitarios, que incluye gesti\u00f3n de casos, derivaciones comunitarias, educaci\u00f3n sanitaria, promoci\u00f3n y visitas domiciliarias, para reducir las tasas de reingreso a la sala de emergencias y aumentar las citas de seguimiento con m\u00e9dicos de atenci\u00f3n primaria.<\/p><p><strong>Estado de la misi\u00f3n<\/strong><br \/>El objetivo general del programa es mejorar la salud y la calidad de vida de los residentes del condado de Tulsa abordando los impulsores sociales de la salud para ayudar mejor a los pacientes a navegar por los servicios sociales y de atenci\u00f3n m\u00e9dica.<\/p><p><strong>Ofertas de programas<\/strong><br \/>La atenci\u00f3n m\u00e9dica puede ser dif\u00edcil de manejar.
