Il utilise une intervention ax\u00e9e sur les agents de sant\u00e9 communautaire, y compris la gestion de cas, les r\u00e9f\u00e9rences communautaires, l&#039;\u00e9ducation sanitaire, le plaidoyer et les visites \u00e0 domicile, pour r\u00e9duire les taux de r\u00e9admission aux urgences et augmenter les rendez-vous de suivi des m\u00e9decins de soins primaires.<\/p><p><strong>\u00c9nonc\u00e9 de mission<\/strong><br \/>L&#039;objectif primordial du programme est d&#039;am\u00e9liorer la sant\u00e9 et la qualit\u00e9 de vie des r\u00e9sidents du comt\u00e9 de Tulsa en s&#039;attaquant aux facteurs sociaux de la sant\u00e9 afin de mieux aider les patients \u00e0 s&#039;orienter dans les soins de sant\u00e9 et les services sociaux.<\/p><p><strong>Offres de programmes<\/strong><br \/>Les soins de sant\u00e9 peuvent \u00eatre difficiles \u00e0 naviguer.
