Ang ating mga Community Health Worker ay ang susi sa pag-abot sa mga populasyong kulang sa serbisyo, pagbibigay sa kanila ng pangangalagang kailangan nila at nagsisilbing tulay para ma-access ang mga kinakailangang serbisyo. Gumagamit ito ng interbensyon na nakatuon sa Community Health Worker, kabilang ang pamamahala ng kaso, mga referral sa komunidad, edukasyon sa kalusugan, adbokasiya at mga pagbisita sa bahay, upang bawasan ang mga rate ng readmission ng Emergency Room at pataasin ang mga appointment sa pag-follow-up ng doktor sa pangunahing pangangalaga.<\/p><p><strong>Pahayag ng Misyon<\/strong><br \/>Ang pangkalahatang layunin ng programa ay upang mapabuti ang kalusugan at kalidad ng buhay ng mga residente ng Tulsa County sa pamamagitan ng pagtugon sa mga social driver ng kalusugan upang mas mahusay na matulungan ang mga pasyente na mag-navigate sa pangangalagang pangkalusugan at mga serbisyong panlipunan.<\/p><p><strong>Mga Alok ng Programa<\/strong><br \/>Maaaring mahirap i-navigate ang pangangalagang pangkalusugan.
