The back to school season is a busy time for everyone, and the Tulsa Health Department is committed to helping make your life easier, happier and healthier! Here is some helpful information about immunizations, shot records, and birth certificates.
Immunizations
Back-to-school immunizations are available on a walk in basis, no appointment necessary. Please bring a current immunization record and your insurance card. For children’s immunizations, a parent or legal guardian must be present. Childhood immunization clinics are available at the following times and locations:

Shot Records
Shot record requests can be made in person at any of the Tulsa Health Department locations or by phone at 918-595-4509. You can also fax a request sent to 918-594-4043.
Birth Certificates
For birth certificates visit the Vital Records department located at 5051 South 129th East Avenue, Tulsa, OK 74134. Birth certificates may be requested in person between the hours of 8:30 am- 4:00 pm at this location. A valid photo I.D. along with payment in cash, check, or money order (made out to the Oklahoma State Department of Health). The cost is $15 per copy except when additional fees apply.
