The Tulsa Health Department’s Health Data & Evaluation division can provide a multitude of health-related data for use in grant applications, presentations, program management and more. Anyone needing a copy of a vaccination record can obtain it online or visit a THD Immunization Clinic for a printed record.

Please note: This applies to vaccines entered in the Oklahoma State Immunization Information System (OSIIS), and may not include all vaccines administered by private physicians, pharmacies, federal and tribal entities.

i.e., I want my medical records from THD like a copy of my STD results or pregnancy testing.

Medical record requests from THD can be made at any of our Tulsa Health Department clinic locations. Alternatively, they may be submitted by filling out and signing our HIPAA Authorization Form and returning to us by email. For the safety of your information, we recommend that you encrypt any emails or password-protect any documents containing sensitive information. Please allow for 5 days to process your request.
