Community partners across Tulsa County can come together to address data-identified health priorities, get involved and take action! The focus of this meeting will be on the State of Chronic Disease Rick Factors and Management.

About Tulsa County CHIP
The Tulsa County CHIP (5 – Year Plan) is a long-term, systematic effort to address public health problems based on the results of Tulsa County’s 2022 Community Health Needs Assessment (CHNA) and build action plans for the improvement in three main health areas: stress & mental health, chronic disease risk factors & management and healthy & affordable housing.
