About Tulsa County CHIP
The Tulsa County CHIP (5 – Year Plan) is a long-term, systematic effort to address public health problems based on the results of Tulsa County’s 2022 Community Health Needs Assessment (CHNA) and build action plans for the improvement in three main health areas: stress & mental health, chronic disease risk factors & management and healthy & affordable housing.
We have 10 locations across Tulsa County that offer a variety of services to help you and your family stay healthy.
