Partnering with local health systems, our Community Health programs provide healthcare and social services to those who need it the most. Our Community Health Workers are the key to reaching underserved populations, providing them with the care they require and serving as a bridge to access the necessary services. This ensures individuals don’t have to navigate complex healthcare systems on their own.
The Center for Community Health is a partnership between Saint Francis Health System and the Tulsa Health Department with the overarching goal to improve health in Tulsa County.

The Community Health Program is a partnership between Ascension St. John and the Tulsa Health Department. It uses a Community Health Worker focused intervention, including case management, community referrals, health education, advocacy and home visits, to reduce Emergency Room readmission rates and increase primary care physician follow-up appointments.
The overarching goal of the program is to improve the health and quality of life of Tulsa County residents by addressing social drivers of health to better help patients navigate healthcare and social services.
