The Tulsa Health Department’s School Health program is proud to be able to partner with elementary schools in Tulsa County.

How to Partner with School Health

THD School Health is proud to partner with Tulsa County elementary schools. Tulsa County elementary schools who are interested in partnering with THD School Health will need to complete an annual School Health Readiness Assessment. Partner schools must also have a district Memorandum of Understanding (MOU). The assessment allows us to gain an understanding of your school’s climate and tailor our services accordingly. Once the assessment has been received and reviewed, a School Health representative will be in contact to discuss possible programming options. School Year Partnership Dates

  • Assessment is closed for 2024-2025 school year.
  • Mark your calendar to complete the assessment for the 2025-2026 school year.
    • Assessment opens May 1, 2025 and closes August 15, 2025
    • Virtual Planning Meetings: May 1 – August 29, 2025

Any assessment submitted after August 15, 2025 will NOT be accepted. Planning meetings must be complete by August 29, 2025 for us to partner with your school for the 2025-2026 school year .

Follow These Steps

Step 1: Review the PDF version of the assessment to ensure all necessary information has been gathered before beginning the online assessment. The information needed to complete the assessment will come from the 2024-2025 school year. If you are unable to answer a question, simply put N/A. We recommend collaborating with the principal, assistant principal, grade level team leads, PE teacher, nurse/health assistant, counselor, and community coordinator/parent liaison to collect the appropriate information and input for the assessment.

Step 2: Select the preferred programming options.

Step 3: Complete and submit the THD School Health Readiness Assessment at the bottom of the page. 

Step 4: Our team will reach out to schedule a virtual planning meeting.

Step 5: Meet with THD School Health staff during the virtual planning meeting to schedule programming for your school.
