The Tulsa Health Department’s Health Data & Evaluation division can provide a multitude of health-related data for use in grant applications, presentations, program management and more. Anyone needing a copy of a vaccination record can obtain it online or visit a THD Immunization Clinic for a printed record.

Please note: This applies to vaccines entered in the Oklahoma State Immunization Information System (OSIIS), and may not include all vaccines administered by private physicians, pharmacies, federal and tribal entities.

i.e., I want my medical records from THD like a copy of my STD results or pregnancy testing.

Medical record requests from THD can be made in person at any of our Tulsa Health Department immunization clinic locations. They can also be completed by filling out and signing our HIPAA Authorization Form and returning to us by email. Please allow for 5 business days to process request.
